What You’ll Do

- Monitoring patient progress and adjusting care plans as necessary.
- Providing education and support to patients and their families.
- Maintaining accurate, up-to-date patient records.
Who You Are
We’re looking for someone who is not only skilled but also passionate about health and helping others. Here’s what we need from you:
- A current RN license and at least 2 years of clinical nursing experience.
- Experience in case management is a big plus, but not required.
- Strong communication skills—you’ll be talking to patients regularly.
- Good organization skills. You’ll manage multiple cases.
- Comfort with technology, as you’ll use software for care management.
